Dr. Donald Ames Person Tripler Army Medical Center "Pacific Basin Telemedicine Consultations and Referrals: Three Years' Experience" Tripler Army Medical Center (TAMC) has pioneered telemedicine initiatives in the Pacific Basin, utilizing developing technologies in support of the Pacific Island Health Care Project (PIHCP). The PIHCP began ten years ago to enhance residency training and to more systematically provide humanitarian care to the medically underserved indigenous peoples of the Unites States Associated Pacific Islands (USAPI). Letters, facsimiles, long distance telephone calls, wide-band interactive teleconsultations and a video telephone were utilized but ultimately abandoned. With Internet access at TAMC and throughout the USAPI, a web-based electronic consultation/referral network was developed. Ten test sites were provided with a desktop computer, digital camera, and scanner. Local providers complete consultations, attach supporting images, and submit them to a secure web page at TAMC. The Project Medical Director reviews the consultations and forwards them to specialists for their recommendations. During the first three years of operation, over 1000 consultations were submitted electronically from American Samoa, Northern Mariana Islands, Guam, Federated States of Micronesia, Marshall Islands, and Palau. By all measures, the Internet can be easily used to provide a store-and-forward solution offering a highly cost effective means for distant consultation, referral, and education. Chan, FASHP Pediatric Research Pharmacist Tripler AMC In-Home Tele-Monitoring of Children with Asthma: An Update of Projects at Tripler Army Medical Center Two in-home pediatric asthma tele-monitoring studies using store and forward technology are being performed at Tripler Army Medical Center in Honolulu, Hawaii. The Telemedicine In-Home Monitoring Evaluation Pilot (TIME-P) project was designed to demonstrate the feasibility of using commercial off the shelf computer technology for in-home monitoring for children with persistent asthma. This pilot study has set the groundwork for our Asthma In-home Monitoring (AIM) project which will determine whether "virtual" asthma visits versus in-person, office-based visits improve adherence to the patient's asthma management regimen and improve disease control. In both studies asthma education and case management is provided in-person or via virtual visit on asthma web site. Adherence is assessed by therapeutic and diagnostic monitoring. Therapeutic monitoring parameters include controller medication use and video of patient using MDI + spacer technique submitted electronically two times a week using store and forward technology. Diagnostic monitoring will include asthma symptom diary and video of peak flow use also submitted electronically using store and forward and file transfer protocol from the web site. Parameters used to assess disease control include lung function tests, quality of life tests, utilization of services, rescue therapy use, symptom control, satisfaction, and asthma knowledge retention.
